Notice

As of January 5, 2024, our office is closed due to the sudden passing of Dr. Benjamin and will no longer be providing any dental services.

It has been a difficult time for all of us. Dr. Benjamin's family and staff wish to thank all of Dr. Benjmain's patients for their support and their many, many messages of condolence. We are truly grateful for this outpouring of appreciation and affection.

In loving memory of
Dr. Gerald C. Benjamin, D.D.S. P.C.
May 4, 1948 - January 6, 2024

Dr. Benjamin's patients likely have many questions, but here are answers to the most common ones:

  1. Can you recommend a new dentist for me?

There are many fine dentists in the Capital Region, and you are free to choose any of them, not just ones we might recommend. We have worked with the following dentists in Saratoga County and can recommend you consider them as your new dentist:

  2. Can you transfer my dental records to my new dentist?

Once you are registered with a new dentist, you will need to submit a "Release of Records Authorization" form to us. We will post an announcement on our web site, https://www.benjaminsmile.com/, when these forms are available. We intend to email the authorization form to those patients for whom we have email addresses.

  3. How can I share my condolences?

    2. To leave a message of condolence for the family online, please visit https://LevineMemorialChapel.com. As you might imagine, we have been gratified but a little overwhelmed by the many emails and phone calls. We do read and very much appreciate all the condolences left on the "tribute wall" on the Levine Memorial Chapel website.
    3. To honor Dr. Benjamin, you may make a contribution in memory of Dr. Gerald Benjamin to a charity of your choice, or The Smile Train, https://www.smiletrain.org/, at PO Box 96246, Washington, D.C. 20090.
